I'll add "Photographing People: Portraits, Fashion, Glamour" from RotoVision, by Roger Hicks, Fraces Schultz, Alex Larg & Jane Wood. Nice hardcover that covers a wide variety of situations and styles from window light to complex studio setups, with overhead and side view lighting diagrams and blurbs on each. I like that it works backwards from real photos by working pros, vs. setting up the shots to illustrate a technique.
