Hi Jeff,

I'm in Australia, so things might be different where you live.  But my 
first thought would be to ask companies who bind theses for university 
students.  Don't know whether they'd be set up to do pages that big, 
though.  But they'd certainly do the quality and quantity you're after - ie 
hardback book bound etc.
