I do not use duotones much..Duotones are used with color inks.  However, I
just read a great article on duotones in this month's Photoshop User
magazine..You should be able to find this magazine in any bookstore..well
worth it...i would definitely subscribe to the magazine.

pretty straight forward B&W mode> RGB mode> Duotone. Then I typically
