Digital BW, The Print

I am trying to print an image that is 11"x 40" on my epson 2200 in Photoshopo 
CS. I remember in the past having problems with the roll feeder driver since 
I'm working in OSX 3, so I'm using the sheet page driver and have created a 
custom size paper. In my Print preview window, the print looks fine - it is 
centered, etc. BUT, when I go to print, it is cutting off the right side (at exactly 
8" by the way). Does anyone have any idea what's wrong? Epson was NO 
help. Thanks.
