Cubes with QTR

I've been using QTR for a while now with great results and few
problems.  This one has me stumped though.  I'm getting 1/2 inch
squares randomly throughout the 13x19 image. Some squares are fine, in
others the image is warped, giving a rather cubist feeling.  Any Ideas?
