The bad 2200 cart I had was the sepia, which I seldom use.  However, I was
making a number of sample prints that included a sepia sample.  I think the
cart emptied the sponge but was unable to replenish it.  The reservoir had
ink in it, but the sponge had become very much lighter than normal -- like
it had been drained of ink.  There was air just above the outlet like you
describe.  I bottom-filled the cart, and it looked and worked fine again --
but just for a short while.  Then the sponge turned light and the nozzle
checks and ink above the outlet disappeared.  The ink was just not feeding
from the reservoir to the sponge.

I put in a new cart, and it worked fine.  So, it was the cart, not the
printer.

Whether not using the sepia regularly is a factor is an obvious question.
Of course, it could just be random.

At any rate, it appears to me that some of these carts can fail, and to
avoid being left unable to print, we need to keep extras -- perhaps empty --
around that we can fill and use if and when a cart fails.  

So far, my failure rate is so low I don't consider it a very serious
problem.  However, if the frequency of the problem gets too high, it
certainly could be a serious issue.  I love these easy-refill and spongeless
carts, so I'm keeping my fingers crossed.
