I'm also getting two QTR choices in the driver dialog for Print (PS CS2) after upgrading to Tiger from 10.3.9. This is for both my 2200 and 4000 printers. I removed and re- installed QTR, but still two QTR items in the dialog. Printing is fine from either so not a problem.
