Yes, here is a copy from the FAQs:

 . . . . . . . . . . .

Do I have to participate in this settlement – can I exclude myself?

Any request to be excluded from the Settlement Class must be made in
writing and mailed, postage prepaid and postmarked no later than June
7, 2006, to the Claims Administrator. Your request for exclusion must
contain: (1) the name of this lawsuit; (2) your full name and current
address; and (3) a statement of intention to exclude yourself from this
lawsuit.
