You can get the PDFs of the mail-in rebates on Epson's site.  The text says:

"Purchase an Epson Stylus Pro 4880 Standard Model, Portrait Edition, or
ColorBurst Edition from an *Epson* *authorized professional imaging reseller
* between August 15, 2009 and October 31, 2009 and recieve $500 back by
mail." Emphasis is mine.

I interpret that as meaning that you have to buy from an authorized
professional imaging reseller, not just any dealer, and the list is on
Epson's professional website.  When I was looking into this, the
professional Imagaing resellers all had info about the rebate on their site,
but Amazon, for instance, did not.  I called Epson Professional Services to
verify this on the phone, and the Epson representative told me that my
understanding was correct.  If you are going to buy from a dealer who is not
an Epson authorized professional imaging reseller, call Epson to verify that
you'll get the rebate.
