Paul, Seth, and John,
I know what gaussian blur is but what is USM? where/how do I access it?

  I wonder if Smart Blur is just an action (a convenience) combining
  gaussian blur and USM? In Elements 2 it's got no adjustments and in
  PS7 its got sliders, instead of "knobs." I wonder if it tends to make
  compromises that gaussian blur followed by USM would not make? Seems
  worth some study.

Filter -> Sharpen -> Unsharp Mask.

Linda, as Paul pointed out, it's  Filter -> Sharpen -> Unsharp Mask   
   ...in Photoshop...CS, 7, and Elements2. 

I suspect it can be more effective to apply USM after gaussian blur,
rather than using smart blur in some situations, but I have to test
that idea further. 

You may want a very low USM threshold, like 8. 

USM is desirable or necessary with most scans, but people often go
overboard with it...that produces telltale white specks and white
halos at edges.

If you are talking photo shop  USM mean unsharp mask.  It is locted under filters and the filter name is unsharp
