First of all: check if the the bus cable connectors are installed correctly. E.g. if only +12V or -12V are applied clipping will occur. Otherwise it sounds like a defective A-138b. There is only one cheap integrated circuit on the pc board (TL062 for version 1, LM324 for version 2) that has to be replaced to solve the problem. I cannot imagine that anything else could cause the fault.
