In answer to your question about midi/cv interfaces that support poly
mode. My Kenton Pro 4 does that.
 
Not sure about their existing range but maybe worth a look.

ach ja. sorry, i was temporarily made stupid by the joy of
anticipation (for want of a better translation of vorfreude) of a poly
midi module.
