Faulty Channel/Voice Card?

Me again, I have experienced this before, but it went away and has now come back for whatever reason. Basically, one of the voices is about 5x loud as the other voices. It looks like it's voice 16 because the relevant LED lights up inside the card cage and the Router Activity on the SC Menu flashes 16 everytime this freak voice occurs... I've made sure all cards are firmly in place! Anyone got any ideas what this could be? Once again, thanks in advance for all help and advice. Regards, Peter
