Check into batchpcb.com ... it is a partner/division of sparkfun.com and focuses on quality, inexpensive PCBs. They have them manufacured off shore and break up large batches of boards (called 'bricks' I seem to remember) to ship them to the purchasers. It isn't super fast, but it is inexpensive and quality from what I can tell. Read the FAQ it should answer your questions.
