In almost all cases, this is due to a bad .quad profile that was made with incorrectly set gray values or the overall gray curve is wonky. Based on just the measurement data, it looks like the Gray_val= settings are wrong, but that is without knowing what inks or printer you are using. (it looks like it would be the K3 inks since there are three spots where the curve has flat spots). Is this a profile you made one that was provided someplace else?
