QTR-Quadtone RIP

QTR Group,

I would greatly appreciate anyone who would take the time to look at the scanned image 
of my "inkseperations.tif" print (with densitometer readings in pencil) and the JPEG capture 
of my "Curve Descriptor File" calibration settings and let me know what I am doing wrong.

I am trying the calibration procedure in OSX and am having serious problems.  I can't 
seem to find anybody to respond to my inquiry… I would greatly appreciate it.  Questions 
concerning the print:

1.  Why aren't ALL the inks printing out?
2.   If only the Black/Grays are supposed to print why do ONLY 2 print instead of 3?
3.  Why does the Black start at 50% and go to 100%?
4.  Why does the LightBlack MAX at 65% and step-down in BOTH directions?

Please help if you can…..
