QTR-Quadtone RIP

I've recently been introduced to QTR after way to many huey Black and white prints. I must 
have spent all day yesterday trying to set it up on my Apple x v. 10.3.9 using an Epson 
2200 printer. I downloaded the software, set up profiles but cannot seem to get the QTR 
print interface in Photoshop cs2. -It's as if it was never installed although my printing 
options now give me the choice for Quad link on epson printer. Sorry but it's driving me 
mad. Any suggestions. I can't even find the file on my hard drive. Below is the info from 
my Diagnostic Script that Mr Harrington said I should include in his tutorial:
