I think I finally found the culprit. At least for me.
Turns out that the curve I created had a space in it. When I dragged it into the profile and placed it in front of the line Grey Curve= it caused QTR to stop functioning properly and made the Quadtone RIP dialog box completely disappear.

In this case, the culprit was the space between "Quad3800" and "files". QTR definitely doesn't like spaces. Anywhere.
I replaced the space with an underscore for the name of the folder I was saving it in ("Quad3800_files" instead of "Quad3800 files", deleted the corresponding .quad file and resinstalled. When I opened Photoshop - BINGO! - it miraculously returned.
