QTR-Quadtone RIP

I am having some trouble with this file, and when I try to print it in
ink-calibration mode, all I get is a large black rectangle.

I can open it in photoshop, and it seems to be OK, but it just doesn't
want to print through QTR calibration mode.  I haven't made any
changes to the file, it's just as it was downloaded.

Any suggestions?  Or, in the alternative, any guidance for simply
building my own custom image for doing the ink-separation image...  is
it just a matter of matching the RGB values for the individual steps
and background, or is there more to the process.
