Calibration, print settings with CS3

The calibration instructions seem to date from the days when 'same as source' was available in print with preview. It appears to be gone in CS3. A few questions on settings: 1. Document setting. Of course this is untagged RGB with the calibration image, but with a picture, for example, a 21-step with known pixel values: Does QTR care (affect print) whether it is untagged, gamma 1.8 or 2.2, gray Lab, dot gain, or whatever, or does this only affect on-screen appearance? 2. Color handling: Absent 'same as source', I assume this must be 'Photoshop manages colors"; with a printer profile of 'QTR - Gray matte paper'? 3. Should one use the same settings for both calibration and profiling? 4. Does it even matter what settings are used provided the same settings are used for printing that were used for profiling?
