QTR-Quadtone RIP

I am having problems making PS CS3 + QTR 2.5.2 center my images on the page when 
printing with my Epson R800. Prints are always off on the vertical dimension by about 1 inch 
on a letter-size sheet.

Note that the printer works well from other applications (Word, etc.). I am using an iMac with 
Leopard.

Has anyone encountered this problem and found a solution?

The R800 in particular is finicky about having the Paper Feed in the program
match the slot where you put the paper.  It still feeds but margins are wrong.

At the best of my knowledge there is only one way to feed sheet paper with the R800, the 
back slot being reserved for roll paper. Is that incorrect> Does QTR expect the paper to be 
fed differently?
