I've added a user conf option for 'Middle C = C4' into the latest build here, so it will appear in the release version. I don't get the logic for C3 as middle C at all, and I can't think of a good way to distinguish C#-1 and C#-2 in 3 characters, so the choice of C4 and C5 will have to do.
