I think Isotonik's template and/or preditor should help you out...
Or use it in CC mode and configure Ableton's remote controller script...

I got extremely excited & I ended up doing it all in about 5 hours . I did 12 channels each with 1 through 64 ,I think it ends up being 64 controls with faders per one preset , I think . Then I assigned each controller P-1 , P-2 main mixer That's P1/1through P-1/128 midi numbers - P-2 that's P2/1 through 2/128 .... Then P3 & P4 for different Instruments & vst controls & so on, looks like this P-3/1through3/128 P-4/1 through 4/128 .....It's real simple to do when you realize that each preset channel can have 128 different commands each & 16 presets & it's all controllable by holding down the edit button & then changing that control to CC & giving it a number under channel 1 through 16 or preset 1 through 16 , they are the same thing  ... Then when you assign on your main mixer say Fader 1 =  1/1 .... Then on preset 2 you have Fader 1 = 2/1 .... Well that Fader on preset two will control something totally different than what Preset Fader 1 controls & they will not bother each other ... Other than that before I realized what was going on I just used Logic Pro Control mode in Mixcraft by clicking setting for controller in Mixcraft & selecting Logic Pro . Having the Emulation mode in Logic . That was the easiest way to use it was in Logic Emulation or Mackie for Cubase Emulation ..... But once I figured out what "Hold down the edit button , while held down , move which control you want to change" It was like a miracle happened !! Ha . I pretty much changed & assigned them while I was mixing a song . Because when you want to use a new VST that is not yet in a preset - just start in any clear preset & hit edit , select its channel & hit learn in your DAW or VSTs midi options  to accept the channel & bam a new control ! Does this make any sense ... This last part I'm trying to say is it can be time consuming , but you can also set up on the fly while using whatever vst or instrument you want to control ..... But like I did , old school , sat there & edited each preset & all of it's channels by preset number & Midi number from 1 to 128 ..... Hope that helps , nothing with midi is really super simple , until you learn & understand midi & what it does & it can still trip you up after you think you have learned everything .... But all you need is that edit button held down & a lot of patience .... There are easier ways , like with Mountain utilities or BCRFEDIT ... But in the end I found the edit & Patience & then save to be the best option for me .... I'm thinking about getting a BCR as well & using my BCF as a Mackie style mixer & The BCR as a multi control unit for the instruments & VSTs .... Hope this helps guys ....
