Cgs synth

Hi,

i have a slight issue with 'CV Cluster' and the frontpanel i got.
I have pots:

- BASE1
- BASE2
-CV MOD1
-CV MOD2

and jacks:

- BASE 1 / BASE 2
- CV MOD 1 / CV MOD 2
- DOWN 1 to 4
- BOTTOM
- TOP
- UP 4-1

do i need 2 x CGS37 ? or can this accomplished with one ?
