Probably a power issue / power-up glitching. You could try adjusting the "DC OK" control (see http://www.rhodeschroma.com/?id=polaris for documentation). You can also try some rapid power cycling NASA-style to see if you can get it to "boot up" correctly and verify the rest of the synth. You might want to measure the power supplies in the unit but be careful, if you blow the DAC you are in trouble!
