Sam, I've always used the Giebler Utilities to do that job.  See 
www.giebler.com  It's not free, but it does the job reliably.  With it, 
you cannot copy an entire Yamaha-protected Pianosoft diskette; however, 
you can copy and convert individual files.

For fun, I once tried the ESEQ2MIDI program and it didn't work for me 
either.  Rather than fight it, I continued to use Giebler which I had 
used for years.

If you have never worked under DOS, you just need to follow Giebler's 
instruction manual.  (If you cut your teeth on CP/M or DOS in the early 
days, you're in for a retro experience.)
