Emax

If your floppy drive is already dead you will need to either get one ready to go (you can 
find them on Ebay or you will need to get an external drive like a Zip and get a 
preformatted Zip cartridge from someone on the list.

If your drive can work long enough all you need to do is run the format utillity Master 5.
