You can not make an OS disk through even a DOS box on a Windows 9x or newer
machine. It has to be through DOS 6.22 or earlier and then it doesn't always work.
The only reliable place I know for replacement drives is http://www.route66studios.com
I'd try getting a OS disk that is known to work before replacing the drive.
