Though not integrated as you want, the ISP1362 OTG is available in 6x6mm BGA, nice and small. One thing I noticed about most embedded USB devices, they are not high speed, just full speed USB. This is true of all ARMs with USB I have looked at. I'm planning to use the ISP1583 to provide 480mb/sec for ATAPI and audio streaming. This chip can connect directly to ATAPI devices.
