Lots available.  I have a Peavey CV1 which is good for control-voltage

Also, check out the Rolls RFX402p which is a stereo expression pedal that 
works for control voltages as well ($45 direct from them, less at some 
dealers like Guitar Center):

As far as pedal switches go, the Fatar and Roland are good.  Check eBay; 
again, most music stores stock these.  Be sure to check if the switch is 
normally open or closed.
