It's called "The CSound Book", editted by Richard Boulanger. I got it at Cody's bookstore in Berkeley, although I'm sure it's widely available. Its chapters are broken down into different CSound applications and are written by people who have done work on the CSound software itself. The introductory chapter is good enough, but I flipped to a later chapter and the writing was crappy-- basically it said "look at the code and run it", although I feel that that particular chapter was an exception.
