How old is your CIS?  If it's more than a year or so, you may have a 
clogged cart(s).  I had problems similar to this and replacing the CIS 
carts solved it.  It seems that the lines and/or the carts can get clogged 
due to evaporation of the black vehicle from the ink.  Nozzle checks can be 
fine, since they have very low ink demand, but printing a 13X19 is a much 
higher demand on ink flow than printing test or a letter size image.  To 
test for ink starvation, print a solid black sheet (letter size will do) at 
*high speed*.  If it begins to show white lines halfway through the sheet, 
you've found your problem.
