I must have missed that in the initial criteria.

Still, it could be done with a gate delay and an EG and maybe a slew 
limiter to soften up the envelope - depending on how you use the 
pitch bend.  If you do not start bending the pitch until after the 
note on gate has occured, you could delay that note on gate and use 
it to start and evelope at some specifc time later.  Of course, I 
think of this primarily from a studio setting when you have multiple 
events coming from a software sequence through a multi-channel 
MIDI/CV.  With a software sequencer and a multi channel CV this 
becomes trivial.  Simply copy the track with the pitchbend to a new 
track and offset the time by what ever value you like.

I do, however, see your point of how the problem becomes more 
complicated when you are dealing with real time performance 
controllers.  Come to think of it, why couldn't you use an envelope 
follower and a VC slew limiter to do this?  The envelope follower 
will basically output the same CV that the real time CV provides, you 
could then use the VC slew limiter to offset the start time.   Maybe. 
maybe not.  I'll have to try this.
