Man, I need to read these things before sending them.  I can hardly
make sense of what I wrote there, and I know what I meant to say.

I believe cakewalk has two program change methods: one where you can
assign a program change to the whole track, and one where you can
change program numbers (send program change messages) while the track
is playing.  It's this second method I think you need to use, because
you have to send two program change numbers, one after the other.

You probably insert the program changes with the event editor to do
this.
