Maxtor XT2190 Jumpers and Formatting

Hi Everyone -

Does anyone have the data for formatting an old Maxtor XT2190 hard drive for
use on one of the early Series IIIs?  I'm particularly in need of the
correct jumper settings for the drive and the adaptec interface board.

Also, any info on the formatting procedure would be great - do I need to low
level format, crbadtrax, etc.
