I know nothing at all about Windows and Qimage.  But my workflow which was set up when I switched to digital has always been: 

Crop and set ratio (4x5, etc., in Raw and convert at 360 ppi (the native dpi output for Epsons)

I size to print size immediately in Photoshop before any post processing.

The idea for me has always been to keep any sizing algorithms out of the picture after any processing has started.

Using the fit to page algorithm is going to put a major pixel adjustment right at the end.  On many images it may not make a difference but it just seems good practise to get those 
major types of alterations done early before subtle adjustments are made. 

As I said, how Qimage interacts with QTR and Windows in terms of placement on the paper is beyond me.
