The MIDI OUT port is the key, because the MD-BT01 parasitically powers itself from the current loop of the host's MIDI OUT port. Wireline MIDI is designed such that an OUT port powers an LED connected to the receiver's IN port. (The LEDs are part of opto-isolators that electrically protect the two MIDI devices from each other.) If the MD-BT01 is connected to only an IN port, there's no source of power, so it won't work.
