Ok, this is how I do it (I use Logic, not PT, but it should be the 
same).

MD gets the midi clock from the sequencer
All the outputs are wired to the audio interface input
The MD sequencer is in "pattern" mode, not "song"
You can still record midi from the MD into the sequencer, but you 
mainly work on audio.

mmmm... not to me.
do you mean that you only have 2 inputs available?
Well, you can still record single tracks on the sequencer, but it's 
going to take you a long time, because you''ll have to use muting on 
the MD.

jack-jack or cannon-jack. it depends on the input of your mixer.
unbalanced because they are cheaper, technically it's the same.
