Two things to check:

Check that you haven't got patch selection set up to activate
from MIDI messages (when certain MIDI events, like the sustain
pedal will change the EXS patch)

Check that you don't have more than one instrument with the same
name in different places. In this case, the EXS can reload the
first matching instrument file found, rather than the actual one
used.
