I had this problem with the Old Lady also from PMI.
It was fixed by opening the preset up in the EXS editor selecting all  
zones and using "update selected zones info from audio file". Then  
saving. To test if this has worked you must unload and reload.  
Everything has been fine since then.

You definitely need to have virtual memory on in the EXS.
