The MacAV use could be the issue. It's very unpredictable with LA, and use of VST instruments wasn't stabilized until I started using a real card. The interim solution is to use the MacAV ASIO driver instead. If you e-mail me directly, I can send you a copy of the driver. It works a lot better. I wish I could give you more conclusive information, but use of OMS and MIDIman gear is pretty dependable.
