Did you do the "Reset Printing System..."  ??   You must clean up
Presets not just delete drivers.

Don't mess with the Epson driver until you have QTR setup.
In fact don't use an image file that you've printed with borderless.
Don't use Photoshop either.
Use TextEdit or Preview to setup QTR Presets.   Make sure when you
select a paper size that
it isn't an Epson driver paper size.

You SHOULD use Presets but they must be initialized starting at
Standard not Last Used or something else.
