I've just begun to get this problem with my 2200 and using QTR (but I really don't use the Epson driver much so can't say if the problem happens there too). First it was just heavier papers HPR 308 etc. but now it has also been happening with EE. Feeding from the straight slot works fine (but is inconvenient). I've tried watching carefully as the feed process starts and it looks like the feeder is very sensitive to the paper being perfectly flat in the Z axis. If there is any bowing then the front sheet won't enter the narrow slot.
