I've done essentially the same thing on my 4000 and can't duplicate a problem. Eric's idea about cutter ON/OFF on the printer may have merit. Do you use both cutter ON & OFF? Are you changing the setting of this on the printer panel? Also are you using the Epson driver immediately prior to QTR? Have you done any other printing? Sheet feeding? Other paper sizes?
