Andrew, thanks.
My question though is this.
Should I continue to install the cups-1.4.5source .tar.gz the same way, in order to have my custom sizes option back?

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
