This used to happen to me. I had a MOTU micro express...or something like that midi interface. MOTU quit updating the drivers for it and it was like my interface gradually became incompatible with my setup. All the note hanging and missing notes that you discribe were totally commonplace. Playback was fine. I switched to an mt4 and all was solved.
