OK, when the Poly goes directly into sequencer edit mode it usually indicates a problem with the chip select Y4 signal getting through to the 3-11A signal.

Have you opened up the Poly and had a look to see if there is any sign of brown sludge that might have come from old batteries in the battery compartment?

The four main pins are nothing. They just ground the outer metal shell of the switch and keep it securely on the board. There are only two pins that matter,
