Ken,

I checked out the DS1 points as recommended below and it ended up being the type of LED used. It was a bluish violet color and when I put a red one in everything was fine. Love the random back and forth sway of the binary lights. To think all the machines we love so much, like this computer I am typing on now, are doing that dance millions of times faster. Neat. Anyone who desires an extinct Blacet Chaos module should own one of your digital noise PCBs. Space Invaders Soundtrack Tool! Makes me think of old Atari 2600 games for example.

You need the common old CMOS version - CD4046 or MC14046 or similar. The 74
variants are designed for a maximum of 5 volts, which is why they go bad.

No. I suggest you either have a bad solder joint, or wrong value resistor,
in particular the 2k2 to ground from the driver transistor.

Grounding as described is perfectly fine. I do exactly the same thing. Other
ground connections are merely for convenience, for those who wish to run
extra wires for whatever reason.
