good evening and many thanks for the tips...I taped up the permanent hole on the floppy and ran the format from the DOS window...the disc changed format to FAT

For those needing it, here are instructions for formatting a HD or DD floppy disk to 720k. If the disk is HD cover the extra hole with adhesive tape.
