IC 13 is the Boot EPROM and is changed with the SCSI update. I believe I have a Non-SCSI EEPROM around somewhere still- but it is most likely you may have bent

that scsi to ide card dont work with older samplers the scsi 1 standard voltage implimentation for termination this card does not provide and wont see older

Have you verified that the SCSI internal HD the termination is OFF? Set it to OFF and set Term on for Jaz. There should only be 1 terminator on the bus.
